Checklist of Requirements

Building Permit

  • Application Letter*
  • Accomplished Building Permit Form
  • Certified True Copy of Land Title and Tax Declaration
  • Duly Notarized Copy of Deed of Absolute Sale, or Lease Contract or Contract to Sell, or Association’s Board Resolution and Applicant’s Certificate of Membership, or Special Power of Attorney (SPA), or Secretary Certificate, or Authority to Construct (whichever is applicable)
  • Certificate of Zoning Compliance
  • Fire Clearance
  • DOLE Clearance
  • DPWH Clearance (if Project is along National Road)
  • ATO Clearance (if Project is near the Airport Area)
  • Bills of Materials
  • Project Specifications
  • Sketch/Lot Plan with Vicinity Map Drawn to Scale
  • Five (5) Sets of Building Plans
  • Structural Design Analysis (for All Buildings/ Structures except for Residential Buildings with an Area of 20 sq.m.)
  • Boring and Plate Load Tests (for Structures Four (4) Stories and Higher)
  • Seismic Analysis (except for Residential Buildings with Height less than 7.5 meters)
  • Construction Logbook

Certificate of Occupancy

  • Application Letter*
  • Zoning Validated Certificate of Occupancy
  • Accomplished Certificate of Completion for:
    • Building Construction
    • Sanitary/ Plumbing Installation
    • Electrical Installation
    • Mechanical Installation
  • Fire Safety Inspection Certificate
  • Picture/s of the Project Showing at least Two (2) Elevations
  • Photocopy of Approved Permits:
    • Building Permit
    • Sanitary/ Plumbing Permit
    • Electrical Permit

Fence Permit

  • Application Letter* with Attached Owner’s valid ID and Three (3) Specimen Signatures
  • Accomplished Fencing Permit Form
  • Certified True Copy of Land Title
  • Duly Notarized Copy of Deed of Absolute Sale or Lease Contract or Contract to Sell or Association Board of Resolution and Applicant’s Certificate of Membership (whichever is applicable)
  • DPWH Clearance (if project is along national road)
  • Details of Fencing Plan

Demolition Permit

  • Application Letter*
  • Accomplished Demolition Permit Forms
  • Certified True Copy of Land Title
  • Notarized Copy of Deed of Absolute Sale or Lease Contract or Contract to Sell or Association’s Board Resolution and Applicant’s Certificate of Membership (whichever is applicable)
  • Sketch/ Lot Plan with Vicinity Map Drawn to Scale
  • Picture of Building to be Demolished
  • Tax Declaration – Land and Building

Signage Permit

Billboard (Advertising Signs)

  • Application Letter* (for New & Renewal)
  • Structural Plan (Signed and Sealed by a Duly Licensed Civil Engineer)
  • Electrical Plan (If Lighted, Plan Signed and Sealed by a Duly Licensed Professional Electrical Engineer)
  • Electronics Plan (for Using Electronics Devices)
  • Structural Analysis (Signed and Sealed by a Duly Licensed Civil Engineer)
  • DPWH Clearance (If the Project is Located along National Highway)
  • ATO Clearance (If the project is within 5km radius of the Airport Premises)
  • Zoning Clearance
  • Contract of Lease (for New and Renewal)
  • Certified True Copy of Land Title
  • Sketch/ Lot Plan with Vicinity Map
  • Previous Permit (for Renewal)
  • Insurance Coverage Policy (for New & Renewal)

Temporary Business Signs (Tarpaulin, Banners, Streamers)

  • List of Private Establishments where the Signage will be Placed, Duly Signed by the Authorized Approving Officer

Electrical Permit

Revocable Electrical Permit

  • Application Form
  • Notarized  Affidavit of Undertaking
  • Clear Picture of the Residential House (Perspective View)
  • Detailed Location Plan
  • Building/ Electrical Inspector Report that the Structure is Made of Indigenous Materials
  • Barangay Certification (for Government and Private Properties)
  • Certification from City Housing (CPDO) that Structure is Indigenous Family Dwelling (for Government Housing and Relocation Projects)
  • Notarized Authorization Letter from the Registered Lot Owner (for Private Properties)
  • Photocopy of Certificate of Land Title (for Private Properties)
  • Photocopy of one (1) Valid ID of the Lot Owner and  One (1) Valid ID of the Applicant, Present All Original IDs (for Private Properties)
  • Certification from Bureau of Fire including Statement of Account from the Davao Light Power Corp. (for Fire Victims)

Reporting Complaints

  • Complaint Letter* specifying the violation/s to the National Building Code of the Philippines
